Job Description

We are seeking compassionate Community Service Workers to join our team in Fort Assiniboine, AB. This role involves supporting children and individuals with intellectual, learning, mental health, and physical disabilities in group homes and supportive living environments. The position focuses on promoting independence, safety, and overall well-being. Permanent, Full-time (30hr/week). To start Immediately Languages: English Education: College, CEGEP or other non-university certificate or diploma from a program of 3 months less than 1 year or equivalent experience Experience: 1 to Less than 7 months experience On site: Work must be completed at the physical location, No option to work remotely. Work Setting: Group home Tasks: Prepare and maintain detailed reports, case notes, and client histories to accurately document client progress, services provided, and any relevant developments. Ensure all documentation meets organizational standards and confidentiality requirements. Provide supervision and guidance to support staff, volunteers, and interns to ensure that services are delivered effectively, professionally, and in accordance with organizational policies and procedures. Assess clients’ needs and determine their eligibility for available programs and support services. This includes reviewing relevant documentation, conducting interviews, and making appropriate referrals when necessary. Perform a variety of administrative duties such as maintaining records, scheduling appointments, coordinating service delivery, and ensuring that required documentation is complete and properly filed. Evaluate clients’ existing skills, personal strengths, and areas requiring development in order to support individualized care plans and promote their independence and well-being. Conduct follow-up assessments with clients to monitor progress, reassess needs, and adjust support strategies or service plans where required. Assist with maintaining a safe, clean, and organized environment by supporting general housekeeping activities within the facility or community setting when necessary. Facilitate conflict resolution by addressing interpersonal issues between clients or staff in a professional and constructive manner, promoting a respectful and supportive living or service environment. Supervise and support the daily activities of clients residing in group homes or halfway houses, ensuring their safety, well-being, and adherence to established guidelines. Provide assistance to clients or guests with special needs, ensuring they receive appropriate care, support, and access to necessary resources. Accompany clients to medical appointments, community programs, and social outings when required, providing supervision and assistance to help them engage safely and confidently in community activities.
